Reservation Request Form

Use this Request Form to request any of the following:

• the use of University facilities
• temporary signage
• a food waiver

Please see our Procedures page for information about bringing food on campus, off-site caterers, grounds usage, and rental policies.

IMPORTANT NOTE:
This form MAY NOT be used for recognized student organizations.  See the student organization section below for more information.

Student Organizations
All requests for rooms, food waivers, or temporary signage must be done through the Dean of Students Office

• You can download and complete the necessary forms at www.utoledo.edu/studentaffairs/stuorgs under "Commonly Used Forms"
• All forms must be turned in to the Office of the Dean of Students (Student Union, Room 2509)
• If your paperwork is not received within 72 hours, your tentative reservation will be cancelled
• If you have any questions regarding these forms, or need guidance in planning your event, please contact the Office of the Dean of Students at 419-530-2256 or studentorganizations@utoledo.edu.
