Change of Graduate Program Form

The Request for Change of Graduate Program form is for students who have already been admitted to a degree program at The University of Toledo and are currently registered. The form is intended:

1. to change a degree being sought, e.g. from masters to PhD or from PHD to masters within the same graduate program, or
2. to change the graduate program of study being sought for currently registered students.
