Rocket Card Access Request

This form will let you request changes, additions, and deletions to the accesses you now have with your Rocket Card.  The accesses granted are based on your student status, employment status, etc. as well as other criteria that may be determined by your instructor, in the case of students, or your supervisor, in the case of employees.

This is a "secure form" on an encrypted website.  If you do not feel safe entering your Employee or Student Identification Number and submitting the form you need to come to our offices in 1610 Rocket Hall during normal business hours.

• If you provide the information requested you could receive a reply by email after the changes you requested have been processed.   Typically this is the following day.

• If the requested changes cannot be made you will be notified of that by email along with a predefined reason why those changes could not be made.

• An missing or incorrect email address will delay processing.
