• $40 nonrefundable admission application fee Apply for admission online and send documents to: Office of Undergraduate Admission, Mail Stop 338, The University of Toledo, 2801 W. Bancroft St., Toledo, OH 43606.
• Official high school transcripts or official GED score and ACT/SAT scores (if taken) (not required for transfer students with 15 or more semester credit hours attempted).
• Official transcripts from ALL colleges/universities attended.
• Proof of Phi Theta Kappa or other honor society membership (if applicable).
• A completed 2010-2011 Free Application for Federal Student Aid (FAFSA) is required for all awards with the exception of the Out-of-State Award and the Phi Theta Kappa Scholarship.

• Award: Variable award amount ranging from $1,000 - $5,000 per year for full-time enrollment. Your award may vary based on merit and financial need.
• Minimum Requirements: 3.0 transfer GPA.
• Additional Documentation Required: A FAFSA is required for scholarship consideration.
• Renewal Requirements: Minimum 3.0 GPA and 30 UT credit hours each academic year. In most instances, the maximum length for this award is four consecutive semesters, not to exceed two consecutive calendar years or completion of an undergraduate degree, whichever comes first.
Note: This award is not stackable with the Out-of-State Award or the Prestige Transfer Scholarship.
• Award: $8,800 per year for full-time enrollment.
• Minimum Requirements: Minimum transfer GPA of 2.75.
• Renewal Requirements: Minimum 2.75 GPA and 30 UT credit hours each academic year. In most instances, the maximum length for this award is four consecutive semesters, not to exceed two consecutive Calendar years or completion of an undergraduate degree, whichever comes first.
Note: This award is not stackable with the UT Achievement Award or the Prestige
Award: Variable award up to $8,000 per year for full-time enrollment. This selective award will be given to top students transferring to UT based on transfer GPA, curriculum, and financial need. Preference is given to honor society members. Funding for this scholarship is limited, competitive, and selective. To be considered for this scholarship, your UT application must be received by May 1, 2010.
• Minimum Requirements: 3.5 transfer GPA; 30 or more semester hours of transfer credit; transferring from a 2-year or 4-year institution.
• Additional Documentation Required: The FAFSA is required for consideration. Proof of honor society membership (if applicable).
• Renewal Requirements: Minimum 3.0 GPA and 30 UT credit hours each academic year. In most instances, the maximum length for this award is four semesters, not to exceed two consecutive calendar years or completion of an undergraduate degree, whichever comes first.

• Award: $1,000 per year, for full-time enrollment.
• Minimum Requirements: 3.75 transfer GPA from the institution of Phi Theta Kappa induction; Phi Theta Kappa membership.
• Additional Documentation Required: Proof of Phi Theta Kappa membership.
• Renewal Requirements: Minimum 3.0 GPA and 30 UT credit hours each academic year. In most instances, the maximum length for this award is four semesters, not to exceed two consecutive calendar years or completion of an undergraduate degree, whichever comes first.
