To make an appointment, please call (419) 383-3030. We will ask you to provide basic information including the client’s name and address, reason for referral, and adaptability levels in the areas of cognitive, communication, behavior, and sensory. We will also provide you with information regarding our diagnostic process and answer any questions you have. Diagnostic assessment appointments are typically scheduled within 2-4 weeks of your call.
