Payment of Educational Fees/Fee Waivers

Instructions: Tuition Waiver form must be filled out in ink (originals not necessary), contain supervisor signature and forwarded to Human Resources AFTER registration in classes. Please make sure a new Tuition Waiver form is completed every semester, per person. Forms can be sent to Human Resources at MS 205 or by fax to 419.530.1492.
Tuition Waivers must be received by the HR Office on or before the last day of the current semester in which the application is for. HR will not process retroactive applications for previous semesters. Human Resources will not be responsible for the removal of any late fees placed on student accounts before submittal of waiver, payment of all fees not covered by the waiver will be the student's obligation.

Main Campus Staff (Non-Faculty)
Main Campus Faculty**:
- Only Full Time Faculty Eligible for use of waiver
- Eligible for payment of 8 credit hours of tuition fee, general fee & all academic fees
- Also eligible for waiver of One time application fee & New Student Registration Fee
Spouses/Domestic Partners of Main Campus Faculty:
- Spouses/DP of MC Faculty hired prior to December 31, 1995 are eligible for payment of undergraduate tuition fee, general fee and all academic fees.
- Spouses/DP of MC Faculty hired after January 1, 1996 are eligible for undergraduate tuition fee only and will be responsible for payment of general fee and all other academic fees for unlimited credit hours
- Spouses/DP also eligible for waiver of one time application fee & new student registration fee.
Dependent Children of Main Campus Faculty:
- Must be under the age 24, unmarried, claimed as IRS dependent on UT employee's taxes and enrolled as a full time student (at least 12 hours)
- Eligible for payment of undergraduate tuition fee only for unlimited credit hours
- Eligible for payment of one time application & new student registration fees
- **All AAUP Faculty members must refer to appropriate collective bargaining agreements for further details about course privledges
Health Science Campus Faculty:
- Applies to all Full Time Faculty and Part Time Faculty of at least .50 FTE
- Full time Faculty eligible for payment of up to 8 credit hours of tuition fee, general fee & academic fees
- Part time Faculty eligible for pro-rated amount according to FTE status up to 8 credit hours of tuition fee & general fee
- .5 FTE will receive, at most, 50% of 8 credit hours of tuition & general fees
- Both are eligible for one time payment of application & new student registration fees
Spouses / Domestic Partners of HSC Faculty:
- Eligible for use of waiver after Faculty has completed one academic year of service
- Eligible for payment of undergraduate tuition fee only for unlimited credit hours
- Eligible for payment of one time application & new student registration fees

Bowling Green State University Reciprocity:
- Only for Full Time Employees NOT Spouse, DPs or Dependent children

In case of the death or retirement of an employee who has completed five years of service with the University, the fee payment entitlement will continue for the surviving spouse, so long as he or she has not remarried, and for any dependent children of decedent or retiree.
CWA, UTPPA, AFSCME and AAUP employees should refer to the appropriate collective bargaining agreement regarding this benefit.
