Family and Medical Leave Act Policy for University of Toledo employees under Policy Number 3364-25-30.

Questions and Answers Regarding Filing for FMLA
Forms need to be completed and turned in 30 days prior to a planned leave.
For an unexpected leave, completed forms must be turned in as soon as possible, not more that  15 calendar days after the leave commences.
Failure to do so may cause the leaves approved start date to be delayed or denied due to untimely notice.

Military Family Leave Forms

Certification for Qualifying Exigency (WH-384)
Complete the information, print, sign and submit the original form to the University of Toledo HRTD department.

Certification for Serious Injury or Illness of Covered Servicemember (WH-385)
Complete the information, print, sign and submit the original form to the various parties necessary to complete the form. The submit the original forms to The University of Toledo HRTD department.

Additional Leaves of Absence available to University of Toledo Employees under Policy Number 3364-25-123 (HSC only).

Military Leave
