• Ohio Public Employee Retirement System (OPERS form):   
  a) Request for Optional Exemption As a Student 
  OR
  b) Enrollment - If you elect to participate in OPERS, you must complete the Personal History Record and the Statement Concerning Your Employment in a Job

• Direct Deposit - Students to complete this online, under MyUT tab

• Personal Information Form  (with VA and War Statement attached)

• Employment Eligibility Verification Form (I-9)
