The following information will assist you in scheduling your submission paperwork.  Upon submission, each application will be pre-reviewed by the IRB office for completeness.  Please be sure to attach all corresponding documents to prevent delays in the review process.  Once all documents are received, an IRB number is assigned and the application is distributed to the primary reviewers for a thorough and substantive review prior to the convened meeting.  In an effort to assure a timely initial or continuing review, please adhere to the dates listed below.  Should you have any questions, please call the numbers listed above or e-mail IRB.Biomed@utoledo.edu

Submission Instructions:

Applications are accepted at any time.  If requesting Continuing Review of Expedited research, please submit one month prior to your expiration date in order to avoid a lapse in approval. Allow at least 10-15 working days for IRB review of new applications.  

 Please submit the following items: 

 ·   1 complete IRB Application with original signatures and an electronic version including the IRB Application, Checklist, Consent Form and all applicable attachments sent to IRB.SBE@utoledo.edu or on a CD.
