Radiation Safety: Reporting Non-Compliance

If you believe you are aware of non-compliance or a breach of policy or procedures regarding Radiation Safety, please follow the steps below:


Step 1. Contact your PI (Lab Director).		If the issue is not resolved, please go to step two.

Step 2. Contact your Radiation Safety Officer.		Main Campus: 419.530.4741 Health Science Campus: 419.383.4301 If not resolved here, go to step three.

Step 3. For the Main Campus, contact Jeff Busch, Institutional Compliance Coordinator.		Main Campus: 419.530.2416
For the Health Science Campus, contact Susan Watterson, Lab Research Compliance Coordinator.		Health Science Campus: 419.383.4252
		If not resolved here, go to step four.
