Preclinical Excused Absence Request Form

This form must be filled out to request an absence from an examination, quiz, required lecture, small group, laboratory, or a or other mandatory course activity in the preclinical years (MS1 and MS2) - (PDF Format) - Download here.

Leave of Absence

This form must be filled out to request a leave of absence for personal, medical, or academic reasons. A minimum of thirty days prior to the start of an LOA is required except under significant extenuating circumstances - (PDF Format) - Download here.
