Officer Listing Form
Deadline: One week after new officers are elected or two weeks after the start of a new semester.



Officer Anti-Hazing Compliance Form
Deadline: Two weeks after the start of each semester.



Roster Change Form
Deadline: Immediately after a student graduates or leaves the chapter. President must verify their roster two weeks prior to the end of fall and spring semesters.



Recruitment Interest Sheet
Deadline: All Potential New Members (PNM) must complete prior to receiving a formal bid and signing the new membership agreement.



Grade Release Coversheet
Deadline: Chapters must submit at the same time the Grade Release Form is submitted.



New Member Education Plan Cover Sheet
Deadline: Two week prior to beginning of recruitment along with new member education plan.
