In order to become a member of the S.W.A.T. Team, fill out the application provided below. The PDF application can be printed and filled out and the Word application can be filled out using Microsoft Word. Upon filling out the application, students may sign up for an interview between February 1 and March 5. Interviews are conducted between the hours of 10 a.m. and 1:45 p.m. To set up an interview call 419.530.3487 and leave a message giving the dates and times that would be best for you. You will be contacted to finalize an interview slot. The advisor letter can be given to your academic advisor as an informational letter to evaluate how S.W.A.T. Team can be applied to your major.
