Graduate Admissions
Admission Forms
For those applicants who are seeking full admission into degree programs, i.e., Certificate, Masters, Education Specialist, DE, Ph.D. (excludes PharmD applicants) or for those applicants seeking admission as a graduate non-degree (special status) student.

For those students who are enrolled at another graduate institution and seeking to take classes at the University of Toledo that are required by their home institution and will be transferred to their home school. Approval from home institution is required. Application form should be submitted to home institution prior to submitting to UT Graduate Admissions.
Graduate Concurrent Enrollment Application (UT/BGSU)
For those students who are enrolled at The University of Toledo or Bowling Green State University and seeking dual enrollment at both institutions. Approval from both institutions is required. Forms are to be submitted to home institution for processing.
For students who have already been admitted to a degree program at The University of Toledo and are currently registered. This form should be completed by students who intend to add certificate to degree program currently enrolled in.
For graduate students who were previously admitted to a graduate degree program, but have been absent for one calendar year and wish to return to complete the degree program to which they were previously admitted. For more information, please see our Readmission Guidelines.
For students who have already been admitted to a degree program at The University of Toledo and are currently registered. Students must complete this form if they intend to change a degree being sought (e.g. from masters to PhD or from PHD to masters within the same graduate program), or to change the graduate program of study being sought.
Financial Guarantee
All international students must also demonstrate that they have adequate financial resources for their graduate education before they can be admitted. Upon recommendation of admission by the department, the College of Graduate Studies will request you provide the necessary financial support documents.
For students who have already been admitted to a degree program at The University of Toledo and are seeking to change their status from provisional admission to regular admission, to change a major in the MBA program, or to change their advisor.
