Work-Study Employment Paperwork Instructions

Once you pick up your Work-Study Contract from LawFinancial Aid, please be sure to complete the following documents and return them to the Office of Professional Development (Suite 1030) as soon as possible.  Please complete only the documents in the list below that apply to you and submit them for review in the order listed.

You are not able to begin working until your paperwork has been processed.

Students Who Are Enrolled in Classes During Work-Study Period

Once your paperwork is processed by Payroll and your “Employee” tab appears in the myUT system, you must electronically complete your W-4 form and Direct Deposit.  You will receive further instructions.

 

**Important Notes for Students Who HAVE PREVIOUSLY PARTICIPATED in Work-Study During Law School:

  • Please contact OPD to determine whether you are required to complete any documents beyond the Work-Study Contract and the Position Information Sheet.
  • If you previously participated in Work-Study during summer, but were not enrolled in classes, you were required to enroll in the Ohio Public Employee Retirement System.  If your new period of employment is occurring during a time when you are enrolled in classes, you are permitted (but not required to) submit the Request for Optional Exemption as a Student form. 

 

 

How to Complete the Required Documents
 

Acknowledgement of Receipt of Auditor of State Fraud Reporting System Information

 

Employee’s Withholding Exemption Certificate (Ohio or Michigan)

  • Ohio = Student must complete all blanks.
  • Michigan = Student must complete blanks 1-9 and date the form.
  • Ohio and Michigan forms are available on the website. 
  • If a student claims exemptions, it may reduce the taxes taken out of each paycheck and may result in additional tax liability depending on the student’s tax status.

* For Ohio, school district codes must be completed and can be found on tax.ohio.gov

 

Form I-9 Employment Eligibility Verification

  • Section 1 – Student must complete all blanks.  If prepared by a person other than the student, complete the “Preparer and/or Translator Certification” section.
  • Section 2 – OPD completes.
  • Section 3 – OPD completes if necessary.

* Student must bring original identification documents to the OPD so that the staff member signing the paperwork on behalf of the university can certify that the documents appear genuine.  Please review the list of acceptable documents on the last page of the I-9 form.  Student must bring:
One document from List A OR
One document from List B AND one document from List C

 

Personal History Record

*Students who are NOT enrolled in classes for summer and are participating in Work-Study must enroll in the Ohio Public Employees Retirement System by completing this form.  10% of your gross wages will be deducted.

  • Section 1 – Student must complete all blanks.
  • Section 2 – Student must complete all blanks; Job Title = Intern
  • Section 3 – Student must complete all blanks.
  • Section 4 – Student must sign and date.
  • Section 5 – Completed by Payroll.

 

Personal Information Form

  • Student must complete all applicable blanks.
  • Must have all 4 pages, can be printed back to back.
  • Write your Rocket # in upper right corner of the top sheet.

 

Position Information Sheet

  • Student must complete all blanks. 

*Supervisor e-mail is especially important since the law school will use this for communications with supervisor.

 

Request for Optional Exemption as a Student (OPERS)
If you are currently registered for classes, you may complete this to request an exemption from wage withholdings for the Ohio Public Employee Retirement System. 

  • Section 1 – Student must complete all blanks.
  • Section 2 – Department = College of Law; Title or Position = Intern; sign and date.
  • Section 3 – Completed by Payroll.

 

Statement Concerning Your Employment in a Job Not Covered by Social Security

  • This must be completed by students enrolling in OPERS.
  • Student completes the following: Employee name; put social security number in “Employee ID #”; sign and date
  • Payroll completes the following: Employer Name and Employer ID
