Health Requirements

All applicants are asked to review the checklist to learn which health requirements are needed to complete. Pay special attention to the Hepatitis B series; this process takes several months.

All admitted and waitlisted students must complete and submit all health requirements to the experiential team by no later than May 8, 2021. 


Candidates for the PharmD program must enter the professional division possessing the essential skills and abilities needed for successful matriculation and performance in a variety of pharmacy practice settings.

Social Security Number Requirement 

If offered provisional admission, a student will be required to have a Social Security number (or ITIN) by May 2021 for the pharmacy intern license application.

If you are a current student and do not have a Social Security number or ITIN, the Office of International Student & Scholar Services can guide you on how to obtain one by the deadline date. Contact the office at 419.530.4229.

If you are not a current UToledo student and you do not have a Social Security number, contact the Social Security Administration office to learn what is required for you to obtain one by the deadline date.

TOEFL Requirement

All international students are required to submit an internet-based TOEFL as part of the application to the PharmD program:

1. Preferred minimum total score of 80 iBT, and
2. Preferred minimum score of 18 in each of the four sub-categories of the iBT (reading, listening, speaking, and writing)
International students from English language proficiency exempt countries (with the exception of the Canadian province of Quebec) are not required to submit official iBT scores.

The daily campus visit schedule begins each day at 10:30 a.m. and again at 2:30 p.m. from Monday through Friday and on Saturday at 11:15 a.m. The schedule is subject to change.
