    Health Science Campus Employees: Completed evaluations must be turned in to the Human Resources satellite office in the Facilities Services Building (FSB), or sent to Mail Stop 1183, by the due date listed below.

    Main Campus Employees: Completed evaluations must be turned in to the Human Resources Office at the Main Campus, Center for Administrative Support, or sent to Mail Stop 205, by the due date listed below.

    Professional Staff and FOP

    All Professional Staff and FOP 4/1 to 3/31 8/31


    (*) Information for CWA Staff Members: Above are the various tools to be used at the present time for all CWA employees. Per the collective bargaining agreement (Article 32.3), “Evaluations will be given to bargaining unit members at least at the mid-point and at the end of their probationary periods. Bargaining Unit employees will be evaluated each year within forty five days following their anniversary date and the evaluation shall be signed by the employee to show receipt. The anniversary date is the date that the employee completes probation. If the evaluations are not completed by the due date, they will be considered null and void and not placed in the employee’s file. Timelines may be extended by mutual agreement between the Union and Management.” Due dates for performance evaluations for CWA employees may be accessed via the web report library (instructions below).

    (*) To Access the Web Report Library:The web report library access request form from the Information Technology Department is located: Web Report Library Access Form. Go to My UT, sign in, go to the employee tab, and scroll down to the bottom of the page to “Web Report Library.” Click that link, click on the green “Production WRL” button on the left-hand side, login, click on the “+” by “Public Folders,” click the “+” next to “Human Resources,” then double click on “General Inquiry.” The report is titled “CWA Annual Review and Longevity.” Double click on the report and add your parameters.
