Performance evaluations are an integral part of the University’s ongoing effort to encourage staff to achieve higher levels of service. Health Science Campus Employees: Completed evaluations must be turned in to the Human Resources drop box located on the Health Science Campus in the Mulford Library Annex outside Room 250, or sent to Mail Stop 205, by the due date listed below.

Main Campus Employees: Completed evaluations must be turned in to the Human Resources Office at the Main Campus, Center for Administrative Support, or sent to Mail Stop 205, by the due date listed below.
