New Hire Benefit Information

Benefit Enrollment Instructions

Please follow these steps to ensure that your benefits are processed:

1.) Submit online enrollment in your myUT portal within 30 days of hire or qualifying event
2.) Submit documentation required for spouse and dependents to benefits@utoledo.edu
   a.) Enrollment/changes are not processed until all documents are received
3.) Submit life insurance form within 30 days of hire
     Main Campus Life Insurance Enrollment Form
     Health Science Life Insurance Enrollment Form
4.) Submit retirement plan election form within 120 days of hire
