2020 Benefits




We have been advised that the Blue Consumer Driven Health  Plan was not accurately set up with Paramount for individuals enrolled in Single +1 or Family Coverage (employees enrolled for single coverage are not affected and no further action is needed for these individuals).  The Blue Plan was set up as an embedded deductible rather than an aggregate deductible.  Paramount is working diligently to update their system and reprocess any affected claims.

For Blue Plan Single +1 and Family participants, please review the below information regarding the possible affects to your medical claims:

  • An aggregate deductible means that instead of each covered person having a single deductible, EVERY covered person will have their claims counted towards the single +1 or family deductible.
    • For example, you have three family members covered under the Blue plan.  One person goes into the hospital and the first $2,800 goes towards the family deductible.  The deductible has now been met for everyone in the family.
    • Another example, one person in your family has $1,000 in claims, another has $1,500 and another has $300.  In this example, the total of the claims is $2,800 therefore the entire family’s deductible has been met.
  • Under an aggregate family deductible, the entire family deductible must be met before the plan will begin paying claims and one individual can meet the family deductible.

Please note that per IRS guidelines, for a Consumer Drive Health Plan to be HSA-qualified (allowing HSA contributions to be pre-tax) it MUST meet the minimum deductible requirements.  Paramount’s plan setup must be corrected for employees to be able to take the deduction on their HSA contributions.

We sincerely apologize for any concern or confusion resulting from this plan error at Paramount.  Please be assured that prior plans, administered by Medical Mutual, were setup and operating properly per IRS rules and regulations.

COVID-19 FLex Spending Account Clarification

In response to the COVID-19 pandemic, many daycares are closed and/or employees are not in need of dependent care.  In response to this changing environment, UToledo will allow employees to complete a qualifying event to reduce or eliminate their dependent care flexible spending account deductions.  Deductions will be adjusted on the next pay cycle following submission of the qualifying event.  Please visit your MyUT portal to submit your qualifying event.  Please use event type “Dependent Status Change”. 

Full-time faculty, unclassified and classified exempt staff and UT police are provided with term life insurance through UNUM. For further details please refer to the below links.

Undergraduate Tuition Waiver 

*Please note the Tuition Waiver Policy is changing for Summer Semester 2020. Please refer to the Benefits homepage for further information.

Employees of The University of Toledo who are eligible to participate in the Tuition Waiver benefit for themselves, spouses, domestic partners or dependents can now submit their waiver online.

The employee can access the Tuition Waiver registration through the MyUT portal on the Employee tab.

You will need to have the following information to complete the Tuition Waiver registration:

  • Your UTAD Username
  • Your UTAD Password
  • Rocket ID Number for each person who will use the tuition waiver
  • Students must be registered prior to submitting the waiver.

Tuition Waiver Registrations must be received by the HR Office on or before the last day of the current semester in which the application is for. HR will not process retroactive applications for previous semesters.  Human Resources will not be responsible for the removal of any late fees placed on student accounts before submittal of waiver, payment of all fees not covered by the waiver will be the student's obligation.

Required Legal Notifications:  UT is required to provide employees the following legal notifications annually. This is for informational purposes only.

Social Security Number/Individual Taxpayer Identification Number Request

Due to recent regulation changes, you must provide a Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN) to the UT Benefits Department for each dependent you wish to enroll in the Plan to satisfy federal reporting requirements. This condition allows UT to comply with a Medicare law requiring health plan administrators to electronically report data for covered plan participants to the Centers for Medicare and Medicaid Services (CMS).

Spouses, same and opposite sex domestic partners are not eligible to begin coverage until a SSN/ITIN has been provided as part of enrollment.  Coverage for dependent children will begin upon enrollment.  However, if a child’s SSN/ITIN is not received within 90 days of enrollment, coverage for the child will be terminated retroactive to the date coverage began.  You may be required to reimburse the Plan for any expenses for which benefits were paid on behalf of an otherwise ineligible dependent.
