Accommodation Request Process
The University of Toledo provides reasonable workplace accommodations to qualified individuals with disabilities to perform the essential functions of their jobs. In accordance with federal law, The Office of Equal Opportunity and Affirmative Action (EOAA) and the Faculty Labor Relations and the Academic Inclusion Office (FLRAIO) have partnered to assist department supervisors throughout the interactive process to identify reasonable workplace accommodations.
ACCOMMODATION REQUEST PROCESS
Submit a completed ADA Accommodation Request Form and supporting documentation

- Employees with a qualifying disability or in need of a COVID-19 related employment accommodation can begin the process by completing the online request form
- Employees must either attach their supporting medical documentation completed by their health care provider or the completed Medical Certification Form

Submitted information is reviewed
After the ADA request form and supporting medical information is reviewed, the Office of EOAA or FLRAIO will contact the employee to schedule an appointment to discuss possible accommodations.
Employing Department Supervisors
The accommodation process is expected to be interactive with participation from the employee and the supervisor. If an ADA reasonable workplace accommodation has been granted by the supervisor to an employee, please contact EOAA or FLRAIO to assist with documenting the implementation of the ADA workplace accommodation.
