Accommodation Request Process For Employees

The University of Toledo, in accordance with the Americans with Disabilities Act (“ADA”), provides reasonable workplace accommodations to qualified individuals with disabilities to perform essential job functions.  In accordance with federal law, the Office of Institutional Equity and HR Compliance processes accommodation requests for all faculty and staff.

Submit a completed ADA Accommodation Request Form and supporting documentation

  1. Employees with a qualifying disability in need of an employment accommodation can begin the process by completing the online request form
  2. Employees should include supporting medical documentation as part of the process.  Employees are encouraged to provide their physician the Medical Certification Form to be submitted as part of the process, though a medical provider may use their own form or letter.
