Dependent Eligibility Verification Assessment Final Deadline
For employees with dependents on the university’s medical plan, the deadline to verify their status with Consova is Friday September 8th. Dependents not verified by September 8th will be removed from the medical plan effective 09.30.23. Refer to the email sent from Consova containing your unique PIN in order to create an account and upload the required documentation.
Leaves of Absence and FMLA Reminders
If an employee needs to be absent from work for a period of time due to a medical reason, they can request a leave under the FMLA. More information can be found on the UToledo Leave of Absence page. Please note, it is the employee’s responsibility to request a leave and provide medical paperwork timely. Notify your direct supervisor about any upcoming leaves at least 30 days in advance or soon as possible.
Please remember to report intermittent usage to FMLA Source and follow department call-off procedures. Salaried employees and faculty should submit Leave Reports to show sick time use.
Prior to returning to work, please provide a Return to Work slip to leaveofabsence@utoledo.edu.
A Supervisor’s responsibility is to advise employees on leave resources available, ensure reporting is accurate, review and approve payroll, and advise employee to contact FMLA Source if they have any questions or issues with their application.
With the pandemic declared as over, anyone with a positive COVID-19 test must apply for an FMLA via FMLASource.com. Positive COVID-19 cases will be treated the same as any other infectious disease.
Retirement Planning Sessions
TIAA will be onsite offering 1:1 consultation sessions from 9a-4p on Wednesdays September 6th and October 11. RSVP as space is limited. Register for sessions at http://www.tiaa.org/schedulenow or by calling 800-732-8353, weekdays, 8 a.m. to 8 p.m. (ET).
