MPH Application

Use the links below to access the SOPHAS application and the UToledo application.  ALL THREE ARE REQUIRED.  When you create a SOPHAS application, you will receive an email from the University with information on how to complete the UToledo Student Conduct Questions form and how to pay the UToledo Graduate Application Fee.


We are currently accepting applications for the  Fall 2023 Semester.
All materials must be uploaded into SOPHAS by July 21st (for international students) and August 4th (for domestic students) to be considered for Fall admission.
NOTE:  We do not accept applications for summer admission.


As of August 15, 2019, the University of Toledo Master of Public Health program began using the SOPHAS application system. SOPHAS allows domestic and international students to apply to multiple institutions using one application.

SOPHAS allows application for single majors only. If you want to pursue a dual major, apply for admission for one major of choice. After admission, complete a Dual Major Declaration form.

UToledo requires an additional fee and student conduct survey.

U.S. STUDENTS: All required documents must be submitted to SOPHAS. Required documents submitted to UToledo other than the supplemental materials (fee & student conduct survey) will not be accepted. 

UTOLEDO CURRENT AND FORMER STUDENTS: UToledo applicants must request that official UToledo transcripts be sent to SOPHAS. UToledo applicants also must request that official transcripts from any other college or university attended also be sent to SOPHAS (that incudes community college transcripts and transcripts representing any transferred courses).

INTERNATIONAL STUDENTSAll required documents must be submitted to SOPHAS  Required documents submitted to UToledo other than the supplemental fee and UToledo student conduct questions form will not be accepted. (NOTE:  The GRE is no longer required)

International Medical Graduates with ECFMG: You should upload a copy of your ECFMG certificate to SOPHAS to waive the TOEFL/IELTS requirement.
